If you've tried gripe water, gas drops, a different swaddle, and a louder sound machine, and your baby is still screaming every evening, the problem probably isn't that you haven't found the right product yet. It's that colic isn't one problem, and most of what you've tried was built to solve only one piece of it.
The research points to three separate root-cause systems, and a baby's colic is usually driven primarily by one of them.
Gut Microbiome Imbalance
A 2020 systematic review in BMJ Open (Ellwood et al.) and a separate 2020 review in Acta Paediatrica (Hjern et al.) both point toward gut-focused intervention as the strongest evidence-based approach for a meaningful subset of colicky infants, particularly breastfed babies. A 2017 network meta-analysis of 32 randomized trials covering 2,242 infants (Gutiérrez-Castrellón et al., published in Medicine) found L. reuteri DSM 17938 outperformed the other interventions studied.
See Probiotics for Colic and Foods That Make Colic Worse for the specifics, including the breastfed-only caveat that matters here.
Nervous System Dysregulation
A newborn's threat-detection system is still calibrating. Startle reflexes, overstimulation, and difficulty self-soothing after being overwhelmed all sit in this bucket.

Feeding Mechanics
Air swallowed during feeding doesn't cause problems at the feed itself. It sits, ferments over two to four hours, and shows up as the evening crying window most parents don't connect back to that afternoon's bottle.

Colic isn't one problem. Most remedies fail because they were built to solve only one of three. Why single-technique fixes work briefly, then stop
Here's why single remedies fail so often: if you're only addressing one of the three systems and a different one is actually primary for your baby, the fix looks like it's "not working," when really it was never aimed at the right target. That's also why gripe water and gas drops fail the way they do. See Colic Myths vs. Facts for the mechanism behind both.
